Summary:
For individuals who install, configure, operate, and troubleshoot medium-size routed and switched networks, including implementation and verification of connections to remote sites in a WAN. This includes basic mitigation of security threats, introduction to wireless networking concepts and terminology, and performance-based skills. Note: On March 26, 2013, Cisco renamed the CCNA certification to CCNA Routing & Switching as part of a program redesign. Previously this was simply known as the CCNA.
Initial Requirements:
You must pass the Cisco Certified Network Associate (CCNA) exam ($295) or pass both the Interconnecting Cisco Networking Devices Part 1 exam ($150) and the Interconnecting Cisco Networking Devices Part 2 exam ($150). Training is available but not required.
The requirements under the new system announced in March 2013 are essentially the same:
If you already hold the Cisco CCENT certification (which means you passed the exam for ICND1), pass the ICND2 exam ($150). Otherwise, pass the CCNA composite exam ($295), which covers both ICND1 and ICND2.

Continuing Requirements:
Certification is valid for three years. Generally you can recertify by passing a specified exam or earning a higher-level certification, but check Cisco's website for the current details as they change from time to time.
